Nurse Practitioner / Physician Assistant – Physiatry (SNF Rehabilitation Program)

Location: Skilled Nursing Facilities

Position Type: Full-Time or Part-Time

Schedule: Flexible weekday rounding

Compensation: $140,000 – $180,000+ earning potential

Position Summary

We are seeking Nurse Practitioners and Physician Assistants to join a growing physiatry-focused rehabilitation program serving skilled nursing facilities. Providers will work with supervising physiatrists and therapy teams to manage rehabilitation patients and optimize mobility and functional recovery. Providers are assigned specific facilities and round several times per week at each location, allowing for continuity of care while maintaining a predictable schedule.T his role offers high clinical autonomy, strong compensation potential, and a flexible rounding structure.

What You'll Do

  • Provide rehabilitation-focused evaluations and follow-up care for patients in skilled nursing facilities

  • Manage conditions such as:

  • Stroke recovery

  • Post-surgical orthopedic rehabilitation

  • Traumatic injury recovery

  • Mobility impairment and functional decline

  • Neuromuscular conditions and spasticity

  • Chronic pain management

  • Develop and coordinate rehabilitation treatment plans with PT, OT, and speech therapy teams

  • Round at assigned facilities multiple times per week

  • Maintain documentation within facility EMR systems

  • Participate in interdisciplinary care planning as needed

Qualifications

  • Licensed Nurse Practitioner (NP) or Physician Assistant (PA)

  • Interest or experience in rehabilitation medicine, geriatrics, hospital medicine, or post-acute care

  • Experience in SNF, inpatient rehab, or hospital consults preferred but not required

  • Strong communication and team collaboration skills
